Begin with the question you want answered
Write down your priorities, the questions you want to discuss, and your preferred language and contact method. For a routine health review, start with previous results or screening questions. Seek local medical care first if symptoms are urgent.
Ask for the receiving team's checklist
Get the institution’s name, the recipient and the transfer channel, then ask the team for a document checklist. It may include consultation summaries, test results, imaging reports, discharge letters and a medicines list. Ask which dates and file formats they need, and whether to arrange translations.
Organise without changing the clinical meaning
Keep the originals and sort readable copies by type and date. Include every page, unit, reference range and institution detail. Mark conflicting reports for the clinician while keeping the original conclusions. Before sending imaging files, ask the team for secure transfer instructions.
Keep originals alongside any translation
Ask which languages and translation formats the team accepts before arranging translation. Keep each translation with its original and label the pair clearly. Preserve medicine names, units and dates, and mark unclear wording. Send the originals along with informal or machine-assisted translations so the receiving team can review them.
Share less, with clear permission
Share the records needed for this enquiry and remove unrelated identifiers and other people’s information. Get permission and appropriate authority before acting for someone else. Read the privacy notice to learn who receives the records, why they are used and how they may be shared further. Communication providers may keep copies. Wait until enquiries open before sending records here, and keep health details out of public links.
Check the transfer and keep your own copy
Use the agreed channel, follow its file type and size limits, and keep your originals. Read any prefilled message before sending it. Ask how you will hear that the records have arrived and that clinical review has taken place. If the status is unclear, contact the recipient before resending sensitive attachments.
Prepare for the next conversation
Keep a list of documents sent and questions still open. Ask who will explain the clinical opinion and arrange follow-up. Contact The Treat for help arranging an appointment, and discuss any proposed change to treatment with your existing medical team.
Common questions
Must I upload medical records for an initial enquiry?
No. Start with a brief question, then ask the receiving team which documents would help. Attachments are optional; wait until enquiries open before sharing information here.
Can I send records on behalf of a family member?
Yes, with their permission and the appropriate authority. Ask which consent or representation documents the institution requires, and share only the relevant records.
Does submitting a report mean my appointment is confirmed?
The institution reviews the relevant records and confirms the appointment separately. Get that confirmation before arranging travel.
Can I ask for deletion or withdraw consent?
You may request deletion or withdraw consent through the published privacy contact when enquiries open. Independent recipients may need separate requests; withdrawal does not undo earlier processing or automatically remove every copy.
