How to pay for healthcare in China: settle four different questions

Start by listing your payment tool, the hospital’s acceptance of it for this bill, the insurer’s agreed contribution and any inpatient advance payment. Tell the cashier which card or mobile wallet you plan to use and ask whether it can pay this bill. Ask your insurer which services your policy covers and what you would pay yourself. Keep each party’s answer.

Use the 2024 edition of the Guide to Payment Services in China, available through the State Administration of Foreign Exchange, to prepare your bank card and mobile wallet. Before visiting Guangzhou United Family Hospital or the Nansha campus of the First Affiliated Hospital, Sun Yat-sen University, give its cashier the campus, bill type and intended payment method and ask about acceptance for this transaction.

Prepare a payment tool before the hospital visit

Download Alipay or WeChat, register or sign in with your mobile number, and add a supported bank card following the app’s prompts. Pay by scanning the merchant’s code or presenting your own. Follow your account’s current instructions; older screenshots may differ.

Before leaving, make sure you can access the registration phone number and receive verification requested by your bank or app. Keep the physical card available if you intend to use it directly. Complete any further verification through the provider’s genuine channel. Once your account is ready, ask the cashier which route can take your particular hospital payment.

The linking options include Visa, Mastercard, JCB, Diners Club and Discover for Alipay; WeChat also includes American Express. Your bank’s approval and the hospital’s acceptance still matter for the card you hold. Open the current app to check your card, then ask the cashier how it can be used for this bill.

Prepare outpatient payment and inpatient advances

Confirm how your chosen payment tool can be used for the actual bill. Outpatient charges and inpatient advance payments may need different arrangements.

Card-network marks and the two wallet routes

For a direct card payment, look for your card network’s acceptance mark at the merchant. If it is absent, ask the cashier. For Alipay or WeChat, add an eligible card and use the payment code. Linking a card and having it accepted directly are separate steps, and acceptance varies by hospital. Tell the cashier which of these routes you intend to use.

[1]

Confirm payment for the outpatient bill

Show the cashier your outpatient bill and identify the foreign card or mobile wallet you want to use. Ask whether it is accepted for this transaction and what alternative is available.

[4]

Prepare an inpatient advance payment

Before admission, ask which identity document and payment evidence are required, how much to prepare and when payment is due. Passport holders can ask international services to help clarify the admission requirements.

[3]

Check how to pay your hospital bill

Identify the campus, bill type and intended payment method when asking about payment. Confirm the arrangements for outpatient and inpatient bills separately.

If the preferred method fails, ask about an accepted backup. A linked overseas card may still be unsuitable for a particular transaction; do not send card details in a general enquiry.

[4]

Keep direct billing separate from the amount you owe

At Guangzhou United Family, the insurer’s payment depends on your plan, and a personal contribution may remain. Contact both parties: ask the insurer about policy cover and the hospital about arranging direct settlement for this visit. Keep a written answer or enquiry reference with your appointment details so you have the arrangement to hand when paying.

Ask which services are included, whether approval is still outstanding and what you may need to pay yourself. Have your policy details ready when you ask, and keep the answer with your appointment records. If you have a second policy, ask the insurer how to use it; the hospital will not automatically split direct billing between both. Chinese public medical insurance eligibility needs its own enquiry, so keep that question separate from the hospital’s billing status.

[2]

Ask about the advance payment before an admission

Nansha’s admission information includes an admission notice, identity verification and an advance payment. Ask which documents apply to you and how much to prepare. If you use a passport, clarify the requirements before admission.

Ask the actual admitting team whether an advance payment applies, when it is due, which payer may make it and what evidence is issued. Ask how it is reconciled with the final account and how any balance is handled. If insurance is being discussed, ask specifically whether a hospital-approved arrangement changes that step. Until the hospital answers, do not assume that expected insurance payment removes the need to prepare funds.

[3]

Plan a backup and keep transaction evidence

Renminbi cash is another payment tool. You can obtain it at an ATM bearing the relevant card-network mark or through an authorised exchange service. Direct card acceptance and mobile payment have different requirements. Ask the cashier where a suitable backup can be used and about that counter’s service hours and transaction limits before deciding how to prepare.

As a practical habit, keep the hospital bill reference beside the bank or wallet transaction reference. If a screen times out, check the deduction status and show the details to staff before repeating payment. If a relative pays, ask how the hospital will associate the payment with the correct patient. Preserve the hospital’s confirmation, not only a screenshot of the payment application.

Prepare the paperwork for a later reimbursement question

If you may pay yourself, ask your insurer in advance which invoices, payment evidence and clinical documents your policy needs, and how to submit them. Requirements depend on the policy. Ask the hospital which documents it can issue and in what form, then find out how to request anything missing after departure.

Keep the advance-payment receipt separate from the final hospital account, and the medical report separate from the transaction record. Before travelling home, list any payment still being reconciled, balance adjustment or missing document. A complete file helps you enquire, while claims and refunds still need their own approval. Write down which hospital or insurer handles each item and ask what you should do next.

Questions to resolve before checkout

  • Which campus, service and cashier are involved in this payment?
  • Has the hospital confirmed my actual card or wallet route and a suitable backup?
  • Have the insurer and hospital separately clarified direct billing and my personal payment?
  • Does an inpatient advance payment apply, and how will it be reconciled?
  • Which final payment records and documents should I retain?

Common questions

Does linking a foreign card prove hospital acceptance?

Linking prepares the wallet; the hospital still needs to accept the actual transaction. Ask its cashier which route works for your card or account.

Can two insurers share the cited hospital’s direct settlement?

Guangzhou United Family offers direct settlement with only one insurer per patient. Ask separately about your other policy and its claims process.

Does insurance automatically remove an inpatient deposit?

Keep the advance payment in your preparation until the admitting team explains the applicable step or a hospital-approved alternative, even if you expect insurance to pay.

What should passport holders prepare for admission?

Prepare the identity document used for the patient record and ask which admission documents and advance-payment evidence are needed. International services can help clarify the process.
